Hospital revenue cycle: how AI can fix errors

Your hospital is losing revenue in silence. This e-book shows the main errors made during the hospital revenue cycle and how to change the course of the story with the help of Rivio’s artificial intelligence solutions

The patient’s claim begins at scheduling. It moves through admission, care, billing and auditing. It reaches the payer, and only then, weeks or months later, does the hospital receive (or not) the amount it should have received from the start.

Every step along this path is an opportunity for error, and every error has a cost.

The 2025 Anahp (National Association of Private Hospitals) Observatory shows that the average time to payment at Brazilian hospitals is above 68 days. The accepted denial rate rose from 0.78% of gross revenue in 2021 to 1.96% in 2024: a deterioration of more than 150% in four years.

Rivio produced this e-book to show clearly where the most common failures in the revenue cycle are, why they keep happening and what hospitals that have already solved this problem did differently.

What you will learn

  • Why do most denials originate before billing, at admission?

  • Which authorization errors especially compromise oncology, where claims can exceed R$ 2 million?

  • How does incorrect documentation on the TISS form generate denial code 3052, one of the most recurrent in the industry?

  • Why has the complexity of payer contracts made manual control unfeasible?

  • How are leading hospitals using AI to audit 100% of claims, cut shelf time from 30 to 7 days and raise the XML acceptance rate from 60% to 95%?

  • What changes in operations when auditing, billing and denial appeals work in an integrated way?

The revenue cycle, step by step

The cycle begins at scheduling: eligibility verification, prior authorization request, plan check. Any failure here spreads through all the following steps.

At admission, a wrong CPF (Brazilian taxpayer ID) or an incorrect health plan code delays submission and opens the door to a denial. During care, procedures not recorded in the medical record and medications not entered become invisible losses. In billing, a discrepancy in amount or code results in immediate rejection by the payer. And after billing, without well-structured clinical documentation, the denial appeal fails and the loss becomes permanent.

The e-book goes through each of these steps with concrete examples and shows where to focus effort to recover revenue.

AI as a structural answer

Artificial intelligence solves the problem when it works end to end: reading medical records, cross-checking clinical records against the hospital claim, applying contractual rules, submitting the XML and generating denial appeals with evidence‑based arguments.

Hospitals that adopted this model began identifying problems during the hospital stay, before discharge, when there is still time to correct them, and reached a 95% XML acceptance rate on first submission, with shelf time cut from 30 to 7 days.

Who is this material for?

For hospital managers, administrative directors, controllers and billing and auditing teams at mid-sized and large hospitals.

Do I need to be a Rivio client to download it?

What sets this e-book apart from other billing materials?

The focus is the revenue cycle as an integrated system, not just an isolated step. The material shows how failures at the start of the process (admission, authorization) turn into financial losses at the end (denials, payment delays), and how AI works at each point along the way.
